With the success of the pneumonia care pathway, Flagler Hospital leaders also deployed a new sepsis pathway. The hospital has expanded its plans for using Ayasdi to develop new care pathways, from the original plan of tackling 12 conditions over three years, to now tackling one condition per month. Future plans are to tackle heart failure, total hip replacement, chronic obstructive pulmonary disease (COPD), coronary artery bypass grafting (CABG), hysterectomy and diabetes, among other conditions. Flagler Hospital expects to save at least $20 million from this program in the next three years, according to officials.

“That was a major effort, but some of us had been data scientists before we were physicians, and so we parameterized all these calls. The first pneumonia care path was completed in about nine weeks. We’ve turned around and did a second care path, for sepsis, which is much harder, and we’ve done that in two weeks. We’ve finished sepsis and have moved on to total hip and total knee replacements. We have about 18 or 19 care paths that we’re going to be doing over the next 18 months,” he says.
The AI tools from Ayasdi revealed new, improved care pathways for pneumonia after analyzing thousands of patient records from the hospital and identifying the commonalities between those with the best outcomes. The application uses unsupervised machine learning and supervised prediction to optimally align the sequence and timing of care with the goal of optimizing for patient outcomes, cost, readmissions, mortality rate, provider adherence, and other variables.

“Every hospital has been struggling with this for decades, managing clinical variation,” he says, noting that traditional methods of addressing clinical variation management have been inefficient, as developing care pathways, which involves identifying best practices for high-cost procedures, often takes up to six months or even years to develop and implement. “By the time you finish, it’s out of date,” Sanders says. “There wasn’t a good way of doing this, other than picking your spots periodically, doing analysis and trying to make sense of the data.”

The premise of our business is that bad people don’t go into healthcare. That’s true even with the big, bad health plans that sometimes get painted into a corner. I’m not suggesting that there aren’t disagreements or even mistrust in healthcare and I’m sure there can be tense moments during contract negotiations between a health plan and a health system. But our job is to find the opportunities where there’s an alignment of incentives. When good people are reminded of why they joined up in healthcare and what their true purpose is, those instincts of competition or mistrust that might lead them to not want to share data fall away. When you give them a cause or a reason to collaborate, people will rally.

Spok researchers were curious whether their predictions became reality, so they analyzed several industry reports and asked a handful of CIOs to recap their experiences from 2018. The most up-to-date responses revealed that compared to last year when just 40 percent of CIOs said they were deploying an enterprise analytics platform in 2018, harnessing data analytics looks to be a huge priority in 2019: 100 percent of the CIOs reported this as top of mind.
In three months, he and his roommate van den Akker, also a computer science student, delivered a web-based application. It included documentation, such as demographics, social determinants, medical history highlights, ER visits and treatment plan. “We tried to minimize the effort hospitals had to expend,” says van den Akker. “If you go after a big data ask, you get pushback.” Still, St. Luke’s didn’t purchase the software. “Hospitals didn’t want to take a bet on two kids in college with no experience in healthcare,” says Green.

Baran says that although automation of repetitive tasks is the ultimate goal, the first step in automation is delegation. “That means shifting the work from physician to staff and using technology to make that process as easy as possible. For the physicians it looks like automation because you are taking this work off their plates, and we use technology to make the process as easy as possible for the staff.”
Klomp, who helped out with strategy while working in private equity at Bain Capital in Boston, quit in 2014 to join Collective Medical. And last year, Benjamin Zaniello, who was a chief medical information officer at Providence Health & Services in Washington, joined as chief medical officer. Zaniello helped implement Collective Medical at Providence. He was impressed. “They did this alone for many years,” he says. “It wasn’t just a bunch of people with a power point and a dream, or someone from Google with a personal story in healthcare who wants to fix the system.”

Anderson, who’s been practicing emergency medicine for 30 years, relies on Collective Medical for 40% of his patients. Recently, a 25-year-old woman showed up in the ER at Auburn Medical Center with abdominal pain. At check-in, an alert popped up next to her name. It was her fifth visit in a year. Her chart showed that she had also been to St. Francis Hospital, Highline Medical Center and Valley Medical Center—all within a 20-mile radius of Auburn. Her prescription drug history revealed that six doctors had ordered narcotics. Anderson contacted her primary care doctor, who was unaware of her ER visits, for a next day appointment, and started her on a treatment for opioid dependence.
Since its founding eight years ago, Collective Medical (not to be confused with employee benefits company Collective Health) has produced a software platform comprised of two main products. EDIE, designed for emergency departments, connects emergency teams across multiple facilities to identify high-risk, complex needs patients and immediately access care history upon admittance. The PreManage product is intended for a wider population of patients, and is marketed to health plans and providers. It also identifies and tracks high-risk patients upon admittance and discharge from inpatient or emergency care, while allowing teams to easily communicate and coordinate throughout a patient’s care.
However, Ganjarunner is much more than an online platform. Baumgartner personally accepts calls for people looking for a custom cannabis regime to ease their suffering. Often, these calls are from cancer and MS patients, but she even receives calls from parents of children with AD(H)D. She works with a doctor to tailor the regimens accordingly. “It’s not about treating the disease, but treating the symptoms it causes,” Baumgartner said.
The company has an intriguing startup story. Fifteen years ago, one of the founders’ mother, Patti Green, was an emergency department social worker in Boise, Idaho, and suspected that some patients were opioid seekers. She set up a rudimentary collaborative care plan for providers to use to identify and help these patients. “It is easy for us now to talk about the opioid epidemic. Nobody was really talking about it 15 years ago, but she was seeing it on the ground,” says Chris Klomp, Collective’s CEO, “and she did something about it.”

Hyderabad: Following the extra vigil by Excise and police sleuths, ganja peddlers in city are delivering ganja to a specific location by charging few extra hundreds. Most of the customers who opt for such services are found to be teenagers with a large peer groups amongst which contacts of these peddlers circulate. Psychiatrists believe that providing excessive cash to young kids may give them an opportunity to opt for such services too.

Our goal now is to invest in the platform and to grow networks. Building network effect-enabled platforms is capital intensive because you need to reach critical density in a given geography to create value for the constituents there. We’ve done a pretty good job of that. We’re live in 17 states, not just with one or two hospitals, but penetrated broadly to 100 percent of acute hospitals. We’ve got a bunch more in the hopper.
One of the ex-ganja peddlers from Fateh Nagar said, `the area is under complete surveillance and many have left such illegal businesses in the locality. However, few of the peddlers continue to sell ganja as they own a vehicle which helps them deliver the drug at a different and preferably safe spot in the city. ``The buyers have the contact details and everything is planned over a phone call”, he said. 

As of January 1st, medical and recreational (adult use) marijuana became legal in the state of California. Per the Prop 64 measure, adults over the age of 21 can now consume, purchase, possess, and grow cannabis without the fear of prosecution so long as they are within the regulation of the new law… and in accordance with any other policies put in place by the city in which they reside.
